ABSTRACT

ObjectiveTo explore the clinical efficacy and safety of Qifu Yixin prescription in treating chronic heart failure in the patients with the syndrome of heart Qi deficiency, so as to provide clinical evidence for the treatment of chronic heart failure with this prescription and promote the clinical application and transformation of this prescription. MethodA total of 106 chronic heart failure patients with the syndrome of heart Qi deficiency who met the criteria in Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine (TCM) from September 2022 to May 2023 were selected and randomized into an observation group (53 cases) and a control group (53 cases). Both groups received routine Western medicine treatment. In addition, the observation group received Qifu Yixin prescription, while the control group received placebo. The treatment course for both groups was 12 weeks. The New York heart association (NYHA) cardiac function grading, N-terminal pro-B-type natriuretic peptide (NT-proBNP), soluble growth-stimulating expression gene 2 (sST2), left ventricular ejection fraction (LVEF), the ratio of early diastolic maximum mitral flow velocity (E) to early diastolic mitral annular motion velocity (e') (E/e'), left ventricular end diastolic diameter (LVEDD), TCM syndrome scores, 6-minute walking test (6MWT), Minnesota Living with Heart Failure Questionnaire (MLHFQ), and Kansas City Cardiomyopathy Questionnaire (KCCQ) were determined before and after treatment. ResultFinally, 102 patients were included for analysis, including 51 patients in the observation group and 51 patients in the control group. After treatment, 42 patients in the observation group showed improved cardiac function grading, with a total response rate of 82.35%, and 32 patients in the control group showed improved cardiac function grading, with a total response rate of 62.75%. The total response rate regarding the cardiac function in the observation group was higher than that in the control group (χ2=4.923, P<0.05). The observation group outperformed the control group in lowering the NT-proBNP level, elevating LVEF, decreasing the E/e' ratio (P<0.05), reducing LVEDD and sST2 levels, and recovering TCM syndrome score, 6MWT score, MLHFQ score, and KCCQ score (P<0.05). None of the safety indexes in the two groups showed abnormal values before and after treatment, and no serious adverse reaction was observed. ConclusionQifu Yixin prescription can improve the heart function, exercise tolerance, and quality of life and alleviate the TCM syndrome of the chronic heart failure patients with the syndrome of heart Qi deficiency by inhibiting myocardial fibrosis.

ABSTRACT

To analyze statistically the situation of the National Natural Science Foundation of China (NSFC) from 2007 to 2016 in the field of acupuncture and moxibustion for supporting the national Universities colleges of traditional Chinese medicine on the General Program (GP) and the National Science Fund for Young Scholars (NSFYS). In view of five aspects, named fund, supporting units, key words, method, disorder and signal path, the differences were compared between GP and NSFYS, the following characteristics were summarized. ① The fund aid was increased from 2007 through 2013 and down-regulated from 2013 through 2016. In recent ten years, the funding condition was fluctuated, but increasing in tendency generally. ② The relevant projects of the same research direction had been approved continuously for over 3 years in a part of TCM universities, in which, the research continuity was the hot topic. ③ Regarding the therapeutic methods, acupuncture was the chief therapy; electroacupuncture, moxibustion and acupoints were involved as well. ④ The disorders involved in the research were cerebral ischemia, myocardial ischemia and reperfusion injury. It is suggested that the ischemic disorder is predominated in the research. ⑤ The signal path occupied the main research index system, including cell proliferation, metabolism, immune, apoptosis and autophagy. The researches on the other aspects were less.

ABSTRACT

Xin'an doctor discussed acupuncture and moxibustion on ' and , calling it "orthodox needling method". 's academic thoughts on moxibustion were mainly reflected in () and (). made an incisive exposition of moxibustion treatment of heat syndrome, ulcer and other issues, and there is a corresponding test case for mutual reference. He believed that the moxibustion method only adapts to sick patients and holds opposition to moth moxibustion care for those who were disease-free; he also advocated that detailed verification of different syndrome should be conducted before acupuncture. This paper expounds 's academic thoughts on moxibustion can be used for heat syndrome, moxibustion does not stay phlegm, moxibustion need syndrome and moxibustion treatment of ulcer. It is believed that can present his own unique insights on the basis of his successors. He had played a guiding and promoting role in the development of academic theory of moxibustion.

ABSTRACT

Objective To establish assay methods for the determination of dissolution,content and related substances of vita-min K1 self-nanoemulsifying drug delivery system(VK1-SNEDDS),and investigate the physico-chemical properties of the preparation. Methods The UV method was established to determine the dissolution of VK1-SNEDDS. The content and related substances were de-termined by HPLC. The appearance,self-emulsification time,micro-morphology,droplet size and zeta potential were also investigat-ed. Results The linearity range of established UV and HPLC methods was 0.85-20.4 and 2.16-216μg/ml,respectively,and all the recovery,precision,specificity and sensitivity met requirements. VK1-SNEDDS could disperse quickly after dilution. The transmission electron microscope(TEM)image of the optimized liquid SNEDDS showed that most of the emulsion droplets were of uniform size with no signs of coalescence. Droplet size of optimal formulation was revealed as 47.74 nm with polydispersibility index(PDI)of 0.248,and zeta potential was found to be-20.53 mV. Conclusion VK1-SNEDDS could form homogeneous and stable nanoemulsion when dilut-ed with aqueous phase and increase the dissolution of lipophilic drug. The methods are reliable,accurate and suitable for quality con-trol of VK1-SNEDDS.

ABSTRACT

Objective To evaluate the diagnostic value of ultrasonography in neonatal necrotizing enterocolitis (NEC)by Meta analysis.Methods PubMed,EMbase,CBM,CNKI,VIP and WanFang databases were retrieved to incorporate studies on the diagnostic value of ultrasound in NEC that met the inclusion criteria.The retrieval time limit was from establishment of the databases to July 24th,2015. QUADAS was used to assess the quality of the literatures and Meta -Disc software (Version 1 .4)was used for Meta analysis.The SROC was drawn and the area under curve (AUC)of SROC was calculated.Results A total of 7 studies were included and 485 patients were involved.The sensitivity of the ultrasonography was 0.65 (95%CI 0.58 ~0.71 )and the specificity was 0.61 (95%CI 0.55 ~0.67).The AUC of the SROC was 0.726.Conclusion The ultrasonography showed no clear advantage for diagnosis of NEC.Combining with clinical manifestations would enhance the diagnostic ability of the ultrasound.

ABSTRACT

Objective To explore the mediating effects of resilience between family function and geriatric depression. Methods By convenient sampling, 212 elderly people who were from the communities in Yuexiu, Guangxhou city were analyzed by using Geriatric Depression Scale (GDS-30) and Family Concern Index Questionnaire (APGAR) and Connor-Davidson Resilience Scale (CD-RISC) and the self-designed General Condition Questionnaire, and a mediating model was proposed and the impacts of resilience on family function and geriatric depression were tested. Results The incidence rate of depression among the community elderly was 23.6% (50/212), and the average score of APGAR was 7.87 ± 2.83, and the average score of CD-RISC was 83.66 ± 12.88. The scores of GDS-30 showed significantly negative correlation with the scores of APGAR (r=-0.582,P<0.01)and the scores of CD-RISC (r=-0.425, P<0.01), and there was positive correlation between the scores of APGAR and the scores of CD-RISC (r=0.335, P < 0.01). The mediating model had high degree of fitting, and level of path had statistical significance (P < 0.01). Resilience was the mediator between family function and geriatric depression. Family function had direct effects (0.50) on geriatric depression and indirect effects (0.11) through resilience, and the combined effects of family function and resilience determined 45%changing range of geriatric depression. Conclusions Family function was a direct predictor of geriatric depression, and resilience had mediating effects between family function and geriatric depression. Community mental health work should pay enough attention to the effects of family function on the life of the aged, and increased the care to the elderly with family dysfunction, while promote and develop family-centered mental health education, propaganda and psychological guidance, so that improves resilience of the aged and help the aged positively coping and maintain good mental health status.

ABSTRACT

Based on the survey on Anhui indigenous medicine and the protection for the traditional Chinese medicine knowledge, we made an analysis and summary for the survey result of Anhui indigenous and special acupuncture techniques in terms of area distribution, types, diseases of treatment, etc. The result shows that though acupuncture and moxibustion are widely applied for all kinds of diseases in most areas of Anhui grassroots regions, they mainly concentrat in major cities like Hefei, Wuhu, Anqing, etc. The techniques are mainly used in orthopedics department and neurology department. They have weighted too much on needling skills, but moxibustion and other special techniques are not paid attention to. We propose that special acupuncture-moxibustion techniques should be promoted, and the relevant training for grassroots medical workers should be developed so as to further enhance their ability to serve grassroots people as well as inherit and innovate the techniques.

ABSTRACT

Objective To investigate the level of self-care behaviors and self-efficacy among adult type 2 diabetes patients in community and discuss current problems.Methods A convenient sample of 85 patients with type 2 diabetes in Haiyuncang community was interviewed,using diabetes self-care scale and diabetes self-efficacy scale.Totally 85 questionnaires were got and underwent analysis.Results The index score of self-care behaviors and self-efficacy among adult type 2 diabetes patients in community were 66.98% and 69.02%.There were no significant difference in self-care behaviors of patients whether they attended diabetes lectures or not.Self-efficacy correlated significantly with self-care behaviors.Conclusions The self-care behaviors and self-efficacy of diabetes patients need to be improved,and diabetes education in community should be emphasized and carried out effectively.

BACKGROUND: Injured nerve can regenerate through end-to-side anastomosis and obtain partial physiological function in animal experiment. As an immuno-suppressivedrug, FK506, which is characterized by promoting neural growth and functional recovery, is paid widely attention to.OBJECTIVE: To investigate the effect of FK506 on functional recovery of peripheral nerve after end-to-side anastomosis.DESIGN: Randomized controlled animal study.SETTING: Department of Operation Surgery, Union Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology.MATERIALS: The experiment was completed in the Department of Operation Surgery, Union Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology in March 2005. Totally 26female Wistar rats were selected and divided into experimental group and control group with 13 in each group.METHODS: Peroneal nerve was transacted on the right side of all the animals, the proximal end of peronieal nerve was turned back and ligated, and the distal end was sutured to the side of tibial nerve with perineurial window. No operation was done on the left side. FK506 was injected in the right tibial muscle (2 mg/kg· d), once a day within two weeks in the experimental group, and saline was used in another group as contrast.MAIN OUTCOME MEASURES: Three months afteroperation, the bilateral common peroneal nerve and the tibial muscle were taken for electrophysiological and histological examination, and the number of common peroneal nerve fiber, area of tibialis anterior muscle fiber of transverse section and muscle wet weight of tibialis anterior muscle were measured. The results were expressed with the ratio between right side and left side (i.e.recovery rate).RESULTS: Totally 26 rats entered the final analysis. ① Results of histological examination: Both ratio of number of nerve fiber and ratio of area of muscle fiber in the experimental group were higher than those in the control group (0.734±0.143, 0.412±0.119; 0.628±0.125, 0.432±0.135; P < 0.01,0.05). ② Results of electrophysiological examination: Recovery rate of action potential, recovery rate of single muscular contractility and recovery rate of tetanus contractility in the experimental group were higher that those in the control group (P < 0.05). ③ Ratio of wet weight in the experimental group was higher than that in the control group (0.765±0.101,0.513±0.116, P < 0.05).CONCLUSION: FK506 injected into targeted muscle can promote growth and functional recovery of peripheral nerve in end-to-side anastomosis.

Objective To study the results of treatment of open fracture of tibia and fibula by external fixation with unilateral multifunction external fixation apparatus(UMEFA) and plaster in different period. In order to further probe into the therapeutic effect and mechanisms of Shou Wu Huan Jing Capsule for treatment of male infertility, 180 patients were investigated. Results showed that the prescription had reliable therapeutic effect with no side - effect, and the effective rate was 93. 89%. Effects of Shou Wu Huan Jing Capsule. on human sperm moment velosity, index of vitality, capillary penetrativity test and sperm hypotonic expansion test were analysed for the application in improving the seminal quality of male infertility patients in culture of human sperm with extract of Shou Wu Huan Jing Capsule. The results revealed that it could promote motor ability and membrane function of sperm. Thus, it is of potential application value for treatment of male infertility and in auxiliary fertility technique as extrinsic additive.

ABSTRACT

Objective:To investigate the balance of Th1/Th2 and Tc1/Tc2 in peripheral whole blood from SLE patients and find out the relationship between disease activity and the balance of type 1 and type 2 cytokines.Methods:Peripheral blood cells from SLE patients(active,n=15,inactive,n=20) were stimulated and normal subjects(n=20) with phorbol myristate acetate and ionomycin(PMA/I) for 4 h,then IFN-? and IL-4 in CD3~+CD8~-T cells(Th) and CD3~+CD8~+T cells(Tc) were detected by recently developed intracytoplasmic cytokine-staining techniques,anti-dsDNA antibodies in serum was detected with enzyme linked immunosorbant assay(ELISA) and immunoglobulin in serum and protein in urine were analyzed with rate nephelometry.Results:In SLE patients with active disease,the percent of IFN-?-secreting Th cells significantly increased compared with SLE patients with inactive disease and normal after in vitro PMA/I stimulation,the percent of IFN-?-secreting Tc cels significantly increased compared with normal,but IL-4-secreting Th cells and Tc cells did not vary significantly.In SLE patients with inactive diseases,only the percent of IFN-?-secreting Tc cells significantly increased compared with control.The percent of IFN-?-secreting Th and Tc cells in patients with anti-dsDNA antibody(+) in serum decreased significantly compared with patients with anti-dsDNA antibody(-).Although the percent of IL-4-secreting Th and Tc cells in patients with aberrantly high level of immunoglobulin had no change,but the mean fluorescent intensify(MIF) of IL-4-secreting Th and Tc cells increased significantly compared with patients with normal level of immunoglobulin.There was no correlation between the percent of IFN-? or IL-4 in Th or Tc cells and the content of protein in patients' urine.Conclusion:The balance between type 1 and type 2 cytokine-secreting T cells is disrupted in SLE patients,especially IFN-?-secreting T cells,which is significantly in favour of Th2 cells.The level of IFN-? or IL-4 in Th or Tc cells varied partly with disease activity,which deserved further study.

ABSTRACT

Objective To study the role of chronic prostatitis(CP)in the pathogenesis and development of IgA nephropathy(IgAN). Methods The incidence rate of CP was compared between the patients with IgAN and the patients with non-IgA mesangial proliferative glomerulonephritis. Furthermore, the clinical and pathological features were compared between the IgAN patients without CP and those with CP. The therapeutic effect of anti-inflammatory treatment in IgAN patients with CP was evaluated. Results Incidence of CP in IgAN patients was 44.7% and that in non-IgA mesangial proliferative glomerulonephritis patients was 21.6%. There was significant difference in the incidence of chronic prostatitis between the two groups. The incidence of glomerular sclerosis and the deposition of immune complexes in vascular wall in IgAN patients with CP were higher than that in patients without CP. The treatment of CP may be beneficial in ameliorating IgAN in some patients. Conclusion Chronic prostatitis is one of the probable pathogenetic factors in IgAN.
